CLAIMS SUBMISSION AND VALIDATION PROCESS. 5.1 Claim Process for Settlement Class Members. To be eligible to receive an award under this Agreement, subject to the Claims review process, Settlement Class Members must accurately and timely complete and submit a Claim Form and deliver that form to the Settlement Administrator. Only one Claim Form may be submitted per Settlement Class Member.
