Compliance with Radiation Ordinance (Cap. 303) Bidders shall declare their offered product(s) the compliance with Radiation Ordinance (Cap. 303) with details as below: I/We confirm that:- (Yes/No*) The offered product does not contain any radioactive substances (RS). (Yes/No*) The offered product is not an irradiating apparatus (IA). If the offered product is IA or contain any RS, I/we confirm/commit to the following:- (Yes/No*) I/We have obtained the necessary license from Radiation Board for selling the offered RS/IA product in Hong Kong, or (Yes/No*) I shall/We will obtain the necessary license from Radiation Board for selling the offered RS/IA product in Hong Kong.
