Common use of Consent Forms Clause in Contracts

Consent Forms. Provide the Reviewing IRB with the site-specific information requested/identified in the customizable sections of the Reviewing IRB’s consent form, for review and approval by the Reviewing IRB, when written informed consent is required. Once the consent form is approved for use by the Relying Institution/Site Investigator(s), the Relying Institution will not, and will require that its Site Investigator(s) not, make any change to the form without obtaining prior approval of that change from the Reviewing IRB.

Provide With respect to Research requiring documentation of consent, a Relying Institution or its Site Investigator(s) or other Personnel will provide the Reviewing IRB with the site-specific information requested/identified customizations to the consent form(s) in the customizable sections of the Reviewing IRB’s consent formaccordance with Section 5.6, for review and approval by the Reviewing IRB, when written informed consent is required. Once the any consent form is forms are approved for use by the Relying InstitutionInstitution’s/Site Investigator(s)Investigator(s)’s/other Personnel’s use, the Relying Institution will not, and will require that its Site Investigator(s) Investigator(s)/other Personnel not, make any change changes to the form forms without obtaining prior approval of that change the changes from the Reviewing IRB.
