Continuity of Care Exceptions. 2.6.6.10.1 Unless requested by the Participant, the FIDA Plan may not transition Participants to a Participating PCP until: 2.6.6.10.1. 1The Participant chooses or is assigned a Participating PCP in accordance with Section 2.8.5 and the Participating PCP is capable of serving the Participant’s needs appropriately; 2.6.6.10.1. 2A Comprehensive Assessment and PCSP are complete; and 2.6.6.10.2 A new PCSP has been drafted and implementation of the new PCSP has begun.
