Contract Certification and Political Contribution Disclosure Form. The Provider Agency is advised of its responsibility to file an annual disclosure statement of political contributions with the New Jersey Election Law Enforcement Commission (ELEC), pursuant to P.L. 2005, c.271, section 3 if the Provider Agency receives Contracts in excess of $50,000 from a public entity in a calendar year. It is the Provider Agency’s responsibility to determine if filing is necessary. Failure to so file can result in the imposition of financial penalties by ELEC. Additional information about this requirement is available from ELEC at (000) 000-0000 or at xxx.xxxx.xxxxx.xx.xx/ Section 3.10 Equal Employment Opportunity.
