Common use of Contract Usage Reporting Clause in Contracts

Contract Usage Reporting. The Contractor shall provide a quarterly usage report by the 10th of the month following each quarter (due: OCT 10th / JAN 10th / APR 10th / JUL 10th). The report must be submitted via email to: xxxxxxxxxxxxxx@xx.xxx and the Contract Liaison listed in Section 17. Each report must contain the name of the worker, position assigned, start and end dates for each assignment, total quantity of hours per assignment, total dollars expended, the State agency/department and the name of the agency contact person. Reported volumes and dollar totals may be checked by the State Procurement Bureau against State records for verification. Failure to provide timely or accurate reports is justification for cancellation of the contract and/or justification for removal from consideration for award of contracts by the State.
