Contractor must cooperate with Quality Review Activities and provide access to their facilities, personnel, and records. 10.2.6. Contractor must provide Individuals access to translated information and interpreter services as described in the Information Requirements section of this Agreement. 10.2.7. Contractor must require subcontractors to notify the Contractor, and the Contractor must notify Great Rivers, in the event of a change in status of any required license or certification. 10.2.8. Contractor must participate in training when requested by Great Rivers. Requests for Great Rivers to allow an exception to participation in required training must be in writing and include a plan for how the required information will be provided to targeted subcontractor staff. 10.2.8.1. Annually, all community behavioral health employees who work directly with individuals must be provided with training on safety and violence prevention topics described in RCW 49.19.030. 10.2.9. Contractor and any subcontractors must comply with state and federal non-discrimination policies, Health Insurance Portability and Accountability Act (HIPAA), and HCA Behavioral Health Data Store Data Dictionary and Data Security Requirements. 10.2.10. Contractor must define a clear process to be used to revoke delegation, impose corrective action, or take other remedial actions if the subcontractor fails to comply with the terms of the Subcontract. 10.2.11. Contractor must require that the subcontractor correct any areas of deficiency in the subcontractor’s performance that are identified by the Contractor, Great Rivers or HCA, as part of a subcontract review. 10.2.12. Contractor must provide written or oral notification no later than fifteen (15) calendar days after termination of a MHCP or CDP/CDPT to Individuals currently open for services who had received a service from the affected BHA in the previous sixty (60) calendar days. Notification must be verifiable in the Individual medical record at the BHA. 10.2.13. Contractor must comply with the Great Rivers’ policy and procedures for utilization of Access to Care Standards for BHOs, Distance Standards, and Access Standards. 10.2.13.1. In accordance with Medicaid benefits being the payer of last resort, Contractor and subcontractors must require the pursuit and reporting of all third party revenue related to services provided under this Agreement. 10.2.16. Contractor must abide by all Grievance and Fair Hearing decisions. 10.2.17. GAIN-SS 10.2.17.1. Contractor and any subcontractors for the provision of behavioral health services must use the DBHR provided GAIN-SS and assessment process that includes the use of the tool and quadrant placement for all individuals age thirteen (13) and above. 10.2.17.2. If the results of the GAIN-SS are indicative of the presence of a co-occurring disorder, this information must be considered in the development of the individualized service plan individualized service plan including appropriate referrals. 10.2.17.3. Documentation of the quadrant placement during the Assessment/Admission process and again upon discharge is input into Great Rivers Avatar system.

10.2.17. Contractor for the provision of behavioral health services must provide encounter data for reporting to Great Rivers.
10.2.18. Contractor’s encounter data is due to Great Rivers by within ten (10) days following the end of the month in which services were provided.
10.2.19. Contractor must re-submit data when rejected by Great Rivers due to errors. Data must be re-submitted within thirty (30) calendar days of the date the error report was produced.
10.2.20. Contractor must require that potential Medicaid Enrollees are offered assistance with accessing enrollment into health plans if the potential Enrollees are uninsured at the time they present for services.
10.2.21. Within thirty (30) days of execution of a Subcontract to perform any function under this Agreement, the Contractor shall notify Great Rivers.
10.2.22. The Contractor shall employ licensed staff and subcontractors to provide Medicaid state plan services as defined within the statement of work this agreement. If the Contractor is not able to provide certain services in the statement of works of this agreement, the Contractor is obligated to contract outside its area and network providers to make the services available. The Contractor must maintain documentation and agreements to show the ability to provide services in the statement of works to its enrollees.
10.2.23. The Contractor must use laboratory testing sites that either:
10.2.23.1. Meet the Clinical Laboratory Improvement Amendments (CLIA) requirements at 42 CFR 493.1 and 42 CFR 493.3, or
10.2.23.2. Possess a valid license under the Medical Test Site law, Chapter 70.42 of the Revised Code of Washington.”
