Conversion due to death of subscriber. Upon the death of the subscriber, coverage under the Agreement shall continue for the surviving dependents for any period for which the premium has been paid. A surviving spouse, if covered under the Agreement, shall become the subscriber upon notice to the Plan of the subscriber’s death.
