Conversion due to ineligibility. A dependent who becomes ineligible for coverage pursuant to the terms set out in the Agreement may apply within thirty (30) days thereafter to continue coverage under this program, as an individual subscriber, without proof of insurability, or under another program of the type for which the dependent then qualifies.
