Common use of Copayments and Other Charges Clause in Contracts

Copayments and Other Charges. i. You are required to pay any Copayments listed in the Schedules attached to this Contract. Copayments are paid directly to the Dentist who provides treatment. Charges for broken appointments and visits after normal visiting hours are listed in the Schedules attached to this Contract. ii. In the event that we fail to pay a Contract Dentist, you will not be liable to that Dentist for any sums owed by us. If you have not received Authorization for treatment from an Out- of-Network Dentist, and we fail to pay that Out-of-Network Dentist, you may be liable to that Dentist for the cost of services.
