Common use of COST REPORT Clause in Contracts

COST REPORT. 10 A. CONTRACTOR shall submit an individual and/or consolidated Cost Report to COUNTY no 11 later than sixty (60) calendar days following termination of this Agreement. CONTRACTOR shall 12 prepare the individual and/or consolidated Cost Report in accordance with all applicable federal, state 13 and COUNTY requirements, GAAP and the Special Provisions Paragraph of this Agreement. 14 CONTRACTOR shall allocate direct and indirect costs to and between programs, cost centers, services, 15 and funding sources in accordance with such requirements and consistent with prudent business practice, 16 which costs and allocations shall be supported by source documentation maintained by CONTRACTOR, 17 and available at any time to ADMINISTRATOR upon reasonable notice. In the event CONTRACTOR 18 has multiple agreements for mental health services that are administered by HCA, consolidation of the 19 individual Cost Reports into a single consolidated Cost Report may be required, as stipulated by 20 ADMINISTRATOR. CONTRACTOR shall submit the consolidated Cost Report to COUNTY no later 21 than five (5) business days following approval by ADMINISTRATOR of all individual Cost Reports to 22 be incorporated into a consolidated Cost Report.

CONTRACTOR shall submit the consolidated Cost Report to COUNTY no later 21 29 than five (5) business days following approval by ADMINISTRATOR of all individual Cost Reports to 22 30 be incorporated into a consolidated Cost Report.

CONTRACTOR shall submit the consolidated Cost Report to COUNTY no later 21 3 than five (5) business days following approval by ADMINISTRATOR of all individual Cost Reports to 22 4 be incorporated into a consolidated Cost Report.

In the event CONTRACTOR 18 has multiple agreements contracts for 21 mental health services that are administered by HCA, consolidation of the 19 individual Cost Reports into a 22 single consolidated Cost Report may be required, as stipulated by 20 ADMINISTRATOR. 23 CONTRACTOR shall submit the a consolidated Cost Report to COUNTY no later 21 than five (5) business 24 days following approval by ADMINISTRATOR of all individual Cost Reports to 22 be incorporated into a 25 consolidated Cost Report.

In the event CONTRACTOR 18 has 13 multiple agreements contracts for mental health services that are administered by HCA, consolidation of the 19 14 individual Cost Reports into a single consolidated Cost Report may be required, as stipulated by 20 15 ADMINISTRATOR. CONTRACTOR shall submit the consolidated Cost Report to COUNTY no later 21 16 than five (5) business days following approval by ADMINISTRATOR of all individual Cost Reports to 22 17 be incorporated into a consolidated Cost Report.

COST REPORT. 10 19 A. CONTRACTOR shall submit an individual and/or consolidated separate Cost Report to COUNTY Reports for Period One Period Two, and Three 20 or for a portion thereof, no 11 later than sixty (60) calendar days following the period for which they are 21 prepared or termination of this Agreement. CONTRACTOR shall 12 prepare the individual and/or consolidated Cost Report in 22 accordance with all applicable federal, state 13 and COUNTY county requirements, GAAP generally accepted accounting 23 principles and the Special Provisions Paragraph of this Agreement. 14 CONTRACTOR shall allocate 24 direct and indirect costs to and between programs, cost centers, services, 15 and funding sources in 25 accordance with such requirements and consistent with prudent business practice, 16 which costs and 26 allocations shall be supported by source documentation maintained by CONTRACTOR, 17 and available at 27 any time to ADMINISTRATOR upon reasonable notice. In the event CONTRACTOR 18 has multiple agreements 28 Agreements for mental health services that are administered by HCA, consolidation of the 19 individual 29 Cost Reports into a single consolidated Cost Report may be required, as stipulated by 20 30 ADMINISTRATOR. CONTRACTOR shall submit an individual and/or consolidated separate Cost Report Reports for Period One, Period Two and Period 16 Three, or for a portion thereof, to COUNTY no 11 later than sixty (60) calendar days following the period 17 for which they are prepared or termination of this Agreement. CONTRACTOR shall 12 prepare the 18 individual and/or consolidated Cost Report in accordance with all applicable federal, state 13 and COUNTY requirements, GAAP and the Special Provisions Paragraph of this Agreement. 14 20 CONTRACTOR shall allocate direct and indirect costs to and between programs, cost centers, services, 15 21 and funding sources in accordance with such requirements and consistent with prudent business 22 practice, 16 which costs and allocations shall be supported by source documentation maintained by 23 CONTRACTOR, 17 and available at any time to ADMINISTRATOR upon reasonable notice. In the event 24 CONTRACTOR 18 has multiple agreements Contracts for mental health services that are administered by HCA, 25 consolidation of the 19 individual Cost Reports into a single consolidated Cost Report may be required, as 26 stipulated by 20 ADMINISTRATOR. CONTRACTOR shall submit the consolidated Cost Report to 27 COUNTY no later 21 than five (5) business days following approval by ADMINISTRATOR ADMINSTRATOR of all 28 individual Cost Reports to 22 be incorporated into a consolidated Cost Report.

In the event 29 CONTRACTOR 18 has multiple agreements for mental health services that are administered by HCA, 30 consolidation of the 19 individual Cost Reports into a single consolidated Cost Report may be required, as 31 stipulated by 20 ADMINISTRATOR. CONTRACTOR shall submit the consolidated Cost Report to 32 COUNTY no later 21 than five (5) business days following approval by ADMINISTRATOR of all individual 33 Cost Reports to 22 be incorporated into a consolidated Cost Report.

CONTRACTOR shall submit the consolidated Cost Report to COUNTY no later 21 28 than five (5) business days following approval by ADMINISTRATOR of all individual Cost Reports to 22 29 be incorporated into a consolidated Cost Report.

CONTRACTOR shall submit an individual and/or consolidated a Cost Report to COUNTY no 11 later than sixty (60) calendar days 30 following termination Period One and Period Two.termination of this Agreement. CONTRACTOR shall 12 prepare 31 the individual and/or consolidated Cost Report in accordance with all applicable federal, state 13 and COUNTY requirementscounty requirements and , GAAP generally 32 accepted accounting principles. and the Special Provisions Paragraph of this Agreement. 14 33 CONTRACTOR shall allocate direct and indirect costs to and between programs, cost centers, services, 15 34 and funding sources in accordance with such requirements and consistent with prudent business practice, 16 35 which costs and allocations shall be supported by source documentation maintained by CONTRACTOR, 17 36 and available at any time to ADMINISTRATOR upon reasonable notice. In the event CONTRACTOR 18 37 has multiple agreements Agreements for mental health services that are administered by HCA, consolidation of the 19 1 individual Cost Reports into a single consolidated Cost Report may be required, as stipulated by 20 2 ADMINISTRATOR. CONTRACTOR shall submit the a consolidated Cost Report to COUNTY no later 21 3 than five (5) business days following approval by ADMINISTRATOR ADMINSTRATOR of all individual Cost Reports to 22 4 be incorporated into a consolidated Cost Report.

COST REPORT. 10 19 A. CONTRACTOR shall submit an individual and/or consolidated a Cost Report to COUNTY no 11 later than sixty (60) calendar days 20 following termination of this Agreement.
