Common use of COST REPORT Clause in Contracts

COST REPORT. CONTRACTOR shall submit an individual and/or consolidated Cost Report for Period One, 31 Period Two and Period Three, or for a portion thereof to COUNTY no later than forty–five (45) 32 calendar days following the period for which they are prepared or termination of this Agreement. 33 CONTRACTOR shall prepare the individual and/or consolidated Cost Report in accordance with all 34 applicable federal, state and COUNTY requirements, GAAP and the Special Provisions Paragraph of 35 this Agreement. CONTRACTOR shall allocate direct and indirect costs to and between programs, cost 36 centers, services, and funding sources in accordance with such requirements and consistent with prudent 37 business practice, which costs and allocations shall be supported by source documentation maintained 1 by CONTRACTOR, and available at any time to ADMINISTRATOR upon reasonable notice. In the 2 event CONTRACTOR has multiple Agreements for mental health services that are administered by 3 HCA, consolidation of the individual Cost Reports into a single consolidated Cost Report may be 4 required, as stipulated by ADMINISTRATOR. CONTRACTOR shall submit the consolidated Cost 5 Report to COUNTY no later than five (5) business days following approval by ADMINSTRATOR of 6 all individual Cost Reports to be incorporated into a consolidated Cost Report.

CONTRACTOR shall submit the consolidated Cost 5 21 Report to COUNTY no later than five (5) business days following approval by ADMINSTRATOR of 6 22 all individual Cost Reports to be incorporated into a consolidated Cost Report.

In the 2 event CONTRACTOR 36 has multiple Agreements for mental health services that are administered by 3 HCA, consolidation of the 37 individual Cost Reports into a single consolidated Cost Report may be 4 required, as stipulated by 1 ADMINISTRATOR. CONTRACTOR shall submit the a consolidated Cost 5 Report to COUNTY no later 2 than five (5) business days following approval by ADMINSTRATOR ADMINISTRATOR of 6 all individual Cost Reports to 3 be incorporated into a consolidated Cost Report.

In the 2 event CONTRACTOR has multiple 11 Agreements for mental health services that are administered by 3 HCA, consolidation of the individual Cost 12 Reports into a single consolidated Cost Report may be 4 required, as stipulated by ADMINISTRATOR. 13 CONTRACTOR shall submit the a consolidated Cost 5 Report to COUNTY no later than five (5) business days 14 following approval by ADMINSTRATOR of 6 all individual Cost Reports to be incorporated into a 15 consolidated Cost Report.

In the 2 event CONTRACTOR has multiple Agreements 36 Contracts for mental health services that are administered by 3 HCA, consolidation of the individual Cost 37 Reports into a single consolidated Cost Report may be 4 required, as stipulated by ADMINISTRATOR. 1 CONTRACTOR shall submit the consolidated Cost 5 Report to COUNTY no later than five (5) business 2 days following approval by ADMINSTRATOR ADMINISTRATOR of 6 all individual Cost Reports to be incorporated into a 3 consolidated Cost Report.

In the 2 event 33 CONTRACTOR has multiple Agreements Contracts for mental health services that are administered by 3 HCA, 34 consolidation of the individual Cost Reports into a single consolidated Cost Report may be 4 required, as 35 stipulated by ADMINISTRATOR. CONTRACTOR shall submit the consolidated Cost 5 Report to 36 COUNTY no later than five (5) business days following approval by ADMINSTRATOR ADMINISTRATOR of 6 all 37 individual Cost Reports to be incorporated into a consolidated Cost Report.

CONTRACTOR 10 shall submit the a consolidated Cost 5 Report to COUNTY no later than five (5) business days following approval by ADMINSTRATOR ADMINISTRATOR of 6 all individual Cost Reports to be incorporated into a consolidated Cost Report.consolidated

CONTRACTOR shall submit an anseparate individual and/or consolidated Cost Report ReportReports for 20 Period One, 31 Period Two and Period Three, or for a portion thereof thereof, to COUNTY no later than forty–five sixty (4560) 32 21 calendar days following the period for which they are prepared or termination of this Agreement. 33 22 CONTRACTOR shall prepare the individual and/or consolidated Cost Report in accordance with all 34 23 applicable federal, state and COUNTY requirements, GAAP and the Special Provisions Paragraph of 35 24 this Agreement. CONTRACTOR shall allocate direct and indirect costs to and between programs, cost 36 25 centers, services, and funding sources in accordance with such requirements and consistent with prudent 37 26 business practice, which costs and allocations shall be supported by source documentation maintained 1 27 by CONTRACTOR, and available at any time to ADMINISTRATOR upon reasonable notice. In the 2 28 event CONTRACTOR has multiple Agreements for mental health services that are administered by 3 29 HCA, consolidation of the individual Cost Reports into a single consolidated Cost Report may be 4 30 required, as stipulated by ADMINISTRATOR. CONTRACTOR shall submit the thea consolidated Cost 5 31 Report to COUNTY no later than five (5) business days following approval by ADMINSTRATOR of 6 32 all individual Cost Reports to be incorporated into a consolidated Cost Report.

COST REPORT.
