Coverage for Prosthodontics Clause Samples

Coverage for Prosthodontics and Orthodontic Services as defined in page 3 and page 4 of aforesaid Delta Dental Service Plan on a 50/50 co‐payment basis after each patient pays a Fifty Dollar ($50.00) deductible per calendar year, up to a One Hundred Fifty Dollar ($150.00) maximum.
