Hospitalization and Medical Benefits. 9.2.1.1 The Board will, during the life of the Agreement, maintain and contribute to the cost of the current hospitalization and medical program for full-time faculty members. The health plan year (HPY) is January through December. Effective January 1, 2016, the University will offer the Community Blue PPO with ECM $2000/$4000 plan. The Board has elected the 80%/20% requirements of Section 4 of the Publicly Funded Health Insurance Contribution Act (Act 152 of 2011), hereinafter referred to as the “Act”, for the immediate future. Accordingly, the University will pay no more than 80% of the total annual costs of the medical benefit plan selected above. Employees will pay 20% of the total annual costs of the medical benefit plan selected above. For purposes of this provision, total annual costs includes the premium or illustrative rate of the medical benefit plan, administrative fees, stop loss reinsurance, fees and taxes mandated by state and federal law, and all employer payments for reimbursement of co-pays, deductibles, payments into health savings accounts, flexible spending accounts or similar accounts used for health care, but does not include the costs of dental and vision and does not include beneficiary-paid co-payments or beneficiary payments into health savings accounts, flexible spending accounts or similar accounts used for health care. The employee’s share of premium or illustrated rates will be paid via payroll deduction. The election by the University under Section 4 and 8 of PA 152 may be made annually. If, during the life of this Agreement, the University fails to elect 80%/20%, by default the University will instead use the “hard caps”; the parties will promptly negotiate the effects of this decision on the bargaining unit.
9.2.1.1.1 The Employer may withhold $2.50 per covered employee per year to help fund wellness activities of the Wellness Committee. The University also agrees to contribute $2.50 per covered employee per year for this purpose. The Wellness Committee will consider and discuss various health care issues and concerns.
9.2.1.1.2 The Union authorizes the Employer to withhold the faculty member’s health contribution limit through payroll deduction. Hospitalization and Medical Benefits. 1. The School Corporation will provide a two-tier plan for group health insurance hereafter referred to as the Traditional Plan and the High Deductible Health Plan (HDHP). A member may elect to enroll his/her spouse and/or dependent children in the hospitalization and medical benefits plan. The Traditional Plan will provide for major medical coverage of 80/20 inside the PPO network and will provide for $500 deductible per person and $1,000 deductible per family.
2. Each member on the Traditional Plan will pay 12% of a single or family premium based on the total cost of the premium as of January 1st preceding the contract year. The premium for the HDHP is one dollar (1) per year.
3. The school corporation shall make available the hospitalization and medical benefits plan defined as High Deductible Health Plan (HDHP) for any member who elects to have this plan. The school corporation will contribute $750.00/1,500.00 each year to the teacher’s Health Savings Account, if there is no additional coverage.
4. The out-of-pocket expenses are $1,000/$2,000 in-network and $1,500/$3,000 out-of- network for the Traditional Plan.
5. The out-of-pocket expenses for the High Deductible Health Plan (HDHP) are $5,000/10,000 in-network and $10,000/20,000 out-of-network.
6. Spouses of members of the school corporation’s hospitalization and medical benefits plan are precluded from enrolling in the school corporation member’s plan, as the spouse’s primary insurance, if the spouse’s employer offers a plan that pays at least 70% of a single employee premium in a hospitalization and medical benefits plan. The plan made available by the spouse’s employer will serve as the primary carrier for the spouse and the school corporation member’s traditional family plan will serve as the secondary carrier for the spouse. Dependents are covered by members of the school corporation’s hospitalization and medical benefit plan.

Hospitalization and Medical Benefits. All existing hospital and medical benefits provided to employees and their families shall be retained and continued in full force and effect.

Hospitalization and Medical Benefits. Active employees and eligible spouses and dependents shall be entitled to hospitalization and medical benefits under the Hospital-Medical Plan in effect on January 1, 2009, except as otherwise modified by this Agreement.
