Common use of Daily Vehicle Use Log Clause in Contracts

Daily Vehicle Use Log. This form is used to report mileage for activities related to the General Revenue grant within the organization's local service area. This form is not for use when travel is outside the organization's service area. The signature of the staff claiming mileage is required and the form must be signed by the Executive Director. Executive Directors must authorize the Daily Vehicle Use Log of their staff providing staff is not related to the Executive Director. Executive Directors sign and date their own Daily Vehicle Use Log and must have their Daily Vehicle Use Log authorized by a member of the Board of Directors when claiming reimbursement of mileage expenses.
