Common use of Designation or Change of Survivor Clause in Contracts

Designation or Change of Survivor. (a) If a Survivor has not been designated on an Account, the Account Owner may designate a Survivor by submitting a Request with the full legal name, Social Security Number, and complete contact information for the Survivor. (b) The Survivor of an Account may be changed by submitting a Request. The Survivor may not be changed by will or codicil.

(c) To effectuate a change in Survivor due to the death of the Survivor, if the Account requires joint approval, the Account Owner will be required to submit a copy of the death certificate, or other legally recognized proof of death, and a Request.
