DETAILED INSTRUCTIONS. This SF1449 solicitation package is your proposal. You are bidding on the Forklifts exactly as specified to be delivered in 90 days to the address in the delivery section. 1. You must return once complete signed copies of this solicitation. SF 1449 must be signed in block 30a. 2. You must fill in your firm fixed price in section 4.1, Pricing Data for Required Quantity. The fixed price is for the exact specifications in this solicitation and any deviations will be rejected. This solicitation is for all or nothing. You must bid the exact Forklifts in the total quantity required. No further discussions will be held so this price must be your best and final offer. 3. You must fill out the paragraph 28, Representations and Certifications and sign the bottom. 4. You must provide the name, phone number and email address of a company representative that can obligate your firm on contracts. 5. The only added material shall be your past performance references. You shall supply at least three references with current names, phone numbers and email addresses. 6. The proposal must have the following as a Volume(s) a) Volume I: Standard Form (SF) 1449 0FFEROR TO COMPLETE BLOCK 12.17.24&30 b) Volume II: Price proposal and Section 4.1. (6) As required by H.7, Certificate of Insurance, offeror shall provide either: (a) A copy of the Certificate of Insurance, or (b) A statement that the Contractor will get the required insurance, and the name of the insurance provider to be used. (1) Customer's name, address, and telephone numbers of customer's lead contract and technical personnel; (2) Contract number and type; (3) Date of the contract award place(s) of performance, and completion dates; (4) Contract dollar value; (5) Brief description of the work, including responsibilities; (6) Comparability to the work under this solicitation; (7) Brief discussion of any major technical problems and their resolution; (8) Method of acquisition (fully competitive, partially competitive, or noncompetitive), and the basis for award (cost/price, technical merit, etc.); (9) Percent turnover of contract key technical personnel per year; and (10) Any terminations (partial or complete) and the reason (convenience or default).

(b) The Business Management/Technical Proposal shall be in two parts, including the following information: (1) A list of the names, addresses and telephone numbers of the owners, partners, and principal officers of the Offeror; (2) The name and address of the Offeror's field superintendent for this project; and (3) A list of the names, addresses, and telephone numbers of subcontractors and principal materials suppliers to be used on the project, indicating what portions of the work will be performed by them. (c) Narrative Description of Work Plan/Methodology- Include the following: (1) Description of work, including how the work will be performed, the number of personnel, and groups per area. Detail the work in chronological order including materials, equipment, concrete work, etc. (2) Logistics Plan specific for the development of this project, indicating how the company will manage the project on and off site, important areas include: access, transportation, personnel hiring, subcontracting, utilities, plan updates, correspondence, etc. (3) Contingency Plan with a detailed description of the possible inconveniences that can arise during the development of the project and the alternative solutions (for example: bad weather, road closures, problems with power supply, problems with personnel, accidents on site, etc.). (4) List of Equipment that will be used in the project and indicate the quantity and if it is owned property or rented.
