Determination of Probationary Condition. If the supervisor determines, on the basis of the evaluative criteria, that the performance of an employee under the supervisor’s supervision is unsatisfactory, the supervisor shall place the employee on a probationary status. The employee shall be given a Letter of Probation (Appendix C) from the supervisor which shall include the following information: 1. Specific areas of performance deficiencies; 2. Recommendations for improvement; and 3. A statement indicating the duration of the probationary period which must be at least thirty
