Common use of Disclosure to Employer Clause in Contracts

Disclosure to Employer. Business Associate may provide Summary Health Information regarding the Individuals in the Plan to Employer upon Employer’s written request for the purpose either (a) to obtain premium bids for providing health insurance coverage for the Plan, or (b) to modify, amend or terminate the Plan. Business Associate may provide information to Employer on whether an individual is participating in the Plan or is enrolled in or has disenrolled from any insurance coverage offered by the Plan.
