Common use of Disposal of PHI Clause in Contracts

Disposal of PHI. Except as provided in paragraph 5.04.02 of this Section, upon termination of this Agreement, for any reason, Contractor shall return or destroy all Protected Health Information received from Physician Practice, or created or received by Contractor on behalf of Physician Practice, at the direction of Physician Practice. Contractor shall request, in writing, Protected Health Information that is in the possession of subcontractors or agents of Contractor.
