Divorce of Dependent Spouse. If a Dependent spouse is divorced from the Subscriber, coverage of such Dependent spouse under this Agreement shall terminate at the end of the last period for which payment was accepted by the HMO. The terminated spouse shall be entitled, by applying within sixty (60) days of such termination, to direct pay coverage of the same type for which the terminated spouse is then qualified at the rate then in effect.
