Documents to LEA. The FSMC shall submit the following supporting documents to the LEA monthly: • Daily meal counts by eligibility category by school site for each program in order to meet claim deadlines. • Daily menu production records by program. • Monthly summary of value of USDA donated foods received. • Monthly summary of revenues from other sales. • Monthly operating statements to the LEA by the fifteenth (15th) of the next month. This statement shall reflect all activity for the previous calendar month. • Invoices that show the FSMC has credited the LEA for the value of all USDA donated foods received for use in the LEA’s food service in the school year, including the value of USDA donated foods contained in processed end products. [7 CFR 250.54] • Evidence of the FSMC’s procurement of processed end products on behalf of the LEA.

B. The LEA is required to submit Form SPI NSLP 1505 FSMC Fin Info each November. The purpose of Form 1505 is to reclassify expenditures paid to the FSMC into three expenditure categories: “Labor,” “Supplies (non-food)” and “Other.” The “Other” category should include the FSMC management fee. The FSMC will provide this information to LEA no later than September 15th for reporting on Form 1505. All supporting documentation shall be made available upon request
