Domestic Partner Coverage. This Contract covers domestic partners of Subscribers as Spouses. If You selected family coverage, Children covered under this Contract also includes the Children of Your domestic partner. Proof of the domestic partnership and financial interdependence must be submitted in the form of:
1. Registration as a domestic partnership indicating that neither individual has been registered as a member of another domestic partnership within the last six months, where such registry exists, or
2. For partners residing where registration does not exist, by an alternative affidavit of domestic partnership.
a. The affidavit must be notarized and must contain the following: • The partners are both 18 years of age or older and are mentally competent to consent to contract; • The partners are not related by blood in a manner that would bar marriage under laws of the State of New York; • The partners have been living together on a continuous basis prior to the date of the application; • Neither individual has been registered as a member of another domestic partnership within the last six (6) months; and
b. Proof of cohabitation (e.g., a driver’s license, tax return or other sufficient proof); and
c. Proof that the partners are financially interdependent. Two (2) or more of the following are collectively sufficient to establish financial interdependence: • A joint bank account; • A joint credit card or charge card; • Joint obligation on a loan; • Status as an authorized signatory on the partner’s bank account, credit card or charge card; • Joint ownership of holdings or investments; • Joint ownership of residence; • Joint ownership of real estate other than residence; • Listing of both partners as tenants on the lease of the shared residence; • Shared rental payments of residence (need not be shared 50/50); • Listing of both partners as tenants on a lease, or shared rental payments, for property other than residence; • A common household and shared household expenses, e.g., grocery bills, utility bills, telephone bills, etc. (need not be shared 50/50); • Shared household budget for purposes of receiving government benefits; • Status of one (1) as representative payee for the other’s government benefits; • Joint ownership of major items of personal property (e.g., appliances, furniture); • Joint ownership of a motor vehicle; • Joint responsibility for child care (e.g., school documents, guardianship); • Shared child-care expenses, e.g., babysitting, day care, school bills (nee...
Domestic Partner Coverage. The District will pay the insurance premiums for domestic partner coverage in the same amounts as required for active employees who are married. For the purpose of Contra Costa Community College District health, dental, and vision benefits, "domestic partnership” shall exist between two persons regardless of their gender, and each of them shall be the "domestic partner" of the other if they both complete, and sign and file with the District the "Affidavit of Domestic Partnership" (Appendix H) which includes the following statements:
20.4.14.1 The two parties have resided together for at least six months and have chosen to share their lives in a committed relationship of mutual caring, similar to marriage, for an indefinite amount of time;
20.4.14.2 The two parties are: not married or legally separated from anyone else, eighteen (18) years or older, not related by blood closer than would bar marriage in the State of California, and mentally competent to consent to contract;
20.4.14.3 Neither individual has had another domestic partner within the prior six (6) months (unless the relationship terminated due to death).
20.4.14.4 The two parties declare that they are each other's sole domestic partner; they are responsible for their common welfare, and are financially interdependent. "Financial Interdependence" means that the domestic partners have entered into a contractual commitment for financial responsibility or have joint ownership of significant assets (such as home, car, bank accounts) and joint liability for debts (such as mortgages and major credit cards). Financial interdependence is demonstrated by meeting at least two of the following conditions:
Domestic Partner Coverage. Domestic partner coverage is available in the health, dental and vision insurance plans through the District
a. Employees must select the appropriate plan type to have their domestic partner included and list them as a dependent.
Domestic Partner Coverage. The District will provide health benefits for qualified domestic partners of unit members to the same extent, and subject to the same terms and conditions, as health benefits are available to eligible dependents of unit members under this Agreement. This coverage is conditioned upon the domestic partner meeting all the criteria of California Family Code Section 297, and upon the unit member presenting the District with proof that a valid declaration of domestic partnership has been filed according to the above Family Code Section or with any local agency registering domestic partnerships.
Domestic Partner Coverage. If an employee chooses to complete and submit an Affidavit of Domestic partnership and sign up for medical and/or dental benefits for his or her domestic partner, the employee shall be subject to federal and state income tax withholding.
Domestic Partner Coverage. A Regular Hire, benefits-eligible employee may enroll a registered domestic partner and/or the children of a registered domestic partner in the County’s benefits plans, including medical, dental, vision, and dependent life insurance under the same conditions that apply to spouses and dependent children. The employee will be responsible for all taxes incurred under rules set by the Internal Revenue Service (IRS) and the Franchise Tax Board regarding imputed income.
Domestic Partner Coverage. Medical and vision coverage shall 25 be extended to nurses’ same-sex domestic partners and dependents. Nurses 26 choosing such coverage shall be advised of the potential tax implications and 27 must complete the appropriate legal affidavits during open enrollment. 28
Domestic Partner Coverage. Employees' domestic partners and their dependents shall be eligible for medical and dental benefits on the same terms as employee's spouses and their dependents. Domestic partners of retirees are not covered unless the domestic partnership commenced prior to the retirement. The domestic partner benefit shall be available only to the extent that District medical and dental plan providers agree to make it available. The District shall not be responsible to obtain additional medical or dental insurance carriers solely for the purpose of offering domestic partner benefits. The provision of benefits to the domestic partners shall be on the following terms and conditions: Domestic partners are two adults who have chosen to share one another's lives in an intimate and committed relationship of mutual caring. A domestic partnership may be established between two persons regardless of their gender.
1. A domestic partnership exists when all of the following occur:
a. Both persons have a common residence.
b. Both persons agree to be jointly responsible for each other's basic living expenses during the domestic partnership.
c. Neither person is married nor a member of another domestic partnership.
d. The two persons are not related by blood in a way that would prevent them from being married to each other in this state.
e. Both persons are at least 18 years of age.
f. Both persons are capable of consenting to the domestic partnership.
g. Both persons have filed a Declaration of Domestic Partnership (Exhibit H) with the Xxxx Valley School District.
h. It has been at least twelve months since either of the two parties has filed a Notice of Termination of Domestic Partnership (Exhibit I) with the Xxxx Valley School District. This prohibition does not apply if the previous domestic partnership ended because one of the partners died or married.
i. The two parties agree to notify the Xxxx Valley School District Human Resource Office if there is a change in the circumstances attested to in the Declaration of Domestic Partnership or if the domestic partnership is terminated.
2. A domestic partnership shall terminate when any of the following occurs:
a. One partner gives or sends the other partner a written notice by certified mail that they are terminating the partnership.
b. One of the domestic partners dies.
c. One of the domestic partners marries.

Domestic Partner Coverage. Domestic partner coverage shall be added to applicable health insurance coverage consistent with carrier requirements.
