Common use of Elections and Revocation Clause in Contracts

Elections and Revocation. ECO may be elected by the Owner by submitting a completed and signed election form to Aetna's Home Office. If the Contract Owner has notified Aetna that the TDA Plan is subject to Title I of the Employee Retirement Income Security Act of 1974 as amended, the Owner must also submit the appropriate joint and survivor annuity waiver and spousal consent form(s) to Aetna at its Home Office. Once elected, this option may be revoked by the Owner by submitting a written request to Aetna at its Home Office. Any revocation will apply only to amounts not yet paid. ECO may be elected only once.
