Eligibility Claim. A notarized eligibility claim, in a standard format to be determined by the Employer in accordance with the provisions of this article for the payment of dependent’s or single rate of allowances, shall be submitted to the Employer when first requesting the allowance, and renewed not less frequently than annually thereafter, normally prior to the fiscal year or where any change in dependents claimed arises.
