Eligibility/Enrollment. For purposes of this Article, “Plan” means any approved health plan in which the employee is enrolled.

If you meet the eligibility requirements as set forth in Appendix A and want to enroll, you must sign and agree to abide by the conditions of the LIFE Program, as explained in this agreement. Your LIFE Provider must give you care that meets your needs across all care settings, 24 hours a day, every day of the year. Your LIFE Provider must establish a written care plan that makes sure your care is appropriately provided to you. You will be expected to actively participate and comply with your care plan. Your effective date of enrollment will be the first day of the calendar month following the date you sign the Enrollment Agreement.

However, Residents are eligible to live in the residence halls without taking classes from June 13, 2016 to June 17, 2016, if they are a 2015-16 academic year resident and are registered for summer classes that begin no later than the week of June 20, 2016. Residents are also eligible to live in the residence halls without taking classes from August 13, 2016 to September 4, 2016, if they took a minimum of an eight-week summer class and have a signed 2015-16 contract to live in University Housing residence halls. Early arrival rates will apply September 5-28, 2016.

Only certain camera-enabled Wireless Devices may be used with Mobile Deposit, as specified by us from time to time. It is your responsibility to provide us with accurate, complete and current information about you and your Wireless Device (including your telephone number, the telecommunications carrier, and your email address), and to notify us immediately if any of this information changes or if service to your Wireless Device is terminated. We reserve the right to refuse or cancel your registration for Mobile Deposit if we cannot verify information about you and/or your Wireless Device and accordingly you agree to all of the terms and conditions set forth in this section with respect to Mobile Deposit.

You are automatically enrolled in the Mobile Check Deposit Service when you enroll in the Mobile Banking Service; however, the Mobile Banking Service is prohibited from being offered to money service businesses or owners or lessors of privately owned automated teller machines. We reserve the right to refuse or cancel your registration for the Mobile Check Deposit Service if we cannot verify information about you and/or your Mobile Device and accordingly you agree to all of the terms and conditions set forth in this Addendum with respect to the Mobile Check Deposit Service.
