Program Eligibility. The COUNTY shall provide eligibility determination for those persons applying for home repair under this Agreement by using the following factors:
1. The applicant is a resident of the CITY; and
2. The total income for all members of the applicant’s household does not exceed 80% of the median income of the Kansas City metropolitan area, as determined by the Secretary of Housing and Urban Development; and
3. The applicant is the homeowner and must have occupied the property as a primary residence for at least six (6) months;
4. The property to be repaired is within the corporate limits of the CITY; and
5. When required, medical need will be substantiated and documented.
Program Eligibility. The COUNTY shall provide the eligibility determination of those persons applying for property maintenance assistance under this Agreement, using the following factors:
1. The total income for all members of the applicant’s household does not exceed 80% of the medium income of the Kansas City metropolitan area, as determined by the Secretary of Housing and Urban Development; and
2. The applicant is the homeowner and occupant; and
3. The property to be repaired is within the corporate limits of the CITY; and
4. The applicant has a letter from the NP Division of the CITY citing a Municipal Code violation; and
5. The homeowner is eligible to receive assistance under this section only once per year unless otherwise approved by the CITY’s contract administrator because of unusual and extenuating circumstances.
Program Eligibility. You will be immediately eligible for and enrolled in the Earth tier of the Program when you open an Account and obtain a Card. You will remain eligible for the Earth tier as long as you maintain your Account in good standing. Your Account is considered in good standing if it is open and not suspended or dormant. To become eligible for the Water, Air, or Fire tiers of the Program, you must meet certain minimum Account activity requirements. On a daily basis, we will monitor your prior 30 days of Account activity to determine whether you have met the requisite eligibility criteria for invitation to a higher tier; monitoring begins the day your account is opened if you have been a customer for less than 30 days . Once you have met the eligibility criteria for a particular tier, we will invite you to enroll in that Rewards tier for a twelve-month period (the “Enrollment Period”). When you receive an invitation, you may choose to either continue in your current tier or enroll in the new tier and pay the applicable Activation Fee. If you enroll in the higher tier, you will not receive a refund on your prior Activation Fee or have access to any unused benefits from your prior tier. To qualify for each tier, you must meet the following, minimum qualifying deposits and minimum qualifying Card transactions requirements during the prior 30 days. Program Eligibility. Electronic Recording mandates a close working relationship as well as mutual trust between the County and the submitting entity. All parties of the Electronic Recording transaction desire to operate and maintain a secure recording system that safeguards parties to recordation from deceit, fraud and forgery. This Memorandum of Understanding outlines the procedures and rules for the trusted relationship between the County and Company to facilitate a safe and secure Electronic Recording relationship. Participation in the Electronic Recording program is voluntary and the decision to do so is a business judgment. There will be no added fees or costs of any kind charged by the County for Electronic Recording.
Program Eligibility. Leave credits may voluntarily be transferred from one or more donating workers to another receiving worker under the following conditions:
1. The receiving worker is a permanent full or part-time worker whose participation has been approved by his/her department head;
2. The receiving worker and/or the worker's family member has sustained a life threatening or debilitating illness, injury or condition. (The department head may require that the condition be confirmed by a doctor's report.);
3. The receiving worker has exhausted all paid time off;
4. The receiving worker must be prevented from returning to work for at least 30 days and must have applied for a medical leave of absence.
Program Eligibility. In order to participate in the program as a recipient or donor, an employee must meet all of the following requirements:
a. Employee must have worked for VCPUSD in a contracted position for at least the two consecutive years immediately prior to participating in the program.
b. Employee status must be certificated, permanent classified, confidential, supervisory or management.
Program Eligibility a. Mandatory Participation - This component of the Program shall provide intervention to permanent teachers who receive a rating of a 2 (Does not meet Standard) on the Final Summative evaluation in two of any domains in two consecutive years as provided in the evaluation article of the Agreement through peer Consulting Teachers. They shall be referred to the Joint Committee by their immediate supervisors for intervention under this program.
b. Non-Mandatory Referral - Teachers receiving a rating of a 2 (Does not meet Standard) in any one of the six domains on the Final Summative Evaluation in any two consecutive years may be referred to the Joint Committee.
(1) The Joint Committee shall have the authority to accept or reject non-mandatory referrals from principals or volunteers. Teacher so referred shall have an opportunity to appear before the Joint Committee prior to its determination regarding such a referral. Notification of acceptance into the PAR program shall occur within thirty (30) calendar days of request or not later than June 1st in the year of referral or request to participate.
(2) If the Joint Committee accepts the recommendation for a referred teacher, participation is mandatory except for the volunteer participating teacher.
c. Self-Referred Permanent Teachers who seek to improve their teaching performance may self-refer to the Joint Committee for intervention under this program.
Program Eligibility. You must have an eligible wireless service plan, have purchased an Eligible Device from T-Mobile under an EIP Agreement, be enrolled in the JUMP Program no later than fourteen (14) days from purchase date of the Eligible Device or at such other times as may be identified, and regularly pay Your monthly JUMP Program charge when due. You must also be current and in good standing with Your (i) PHP Terms; and (ii)
Program Eligibility. 1. I meet the educational/experience requirements for participation in the applicable IRTA Program Traineeship program and will provide verification as required.
2. I am a U.S. citizen or resident alien. I will provide acceptable proof of my citizenship or resident alien status at the time I report to activate my Traineeship.
3. I understand that I may have to complete a medical evaluation as directed by the Occupational Medical Services procedures.
Program Eligibility. 1. All officers, regardless of assignment, will be eligible for the vehicle program subject to the limitations set forth below.
2. With the following exceptions, an officer’s domicile must be in Xxxxxxxxxx County to be eligible for the PPV program.
a. Officers in the Centralized Tactical Section and Canine Section, who live within 15 miles of the County line, shall be assigned a vehicle for “to and from” use only.

3. Officers will become eligible for permanent vehicle assignment after satisfactorily completing probation in the PO I rank. Vehicles will be assigned to eligible officers as the vehicles become available. Vehicles shall not be assigned to any officers who reside out of County unless the list of eligible officers who reside in County is satisfied.
4. All officers will be assigned marked police vehicles with the below-listed exceptions. This list may be changed upon the mutual agreement of the department and the union.
