Employees seeking crisis leave with pay shall discuss the circumstances giving rise to the application for such leave with their team leader.
Employees seeking or maintaining, employment in multiple positions within the Gallery, shall notify their Supervisor(s) of their commitments for purposes of scheduling and coordinating hours of work to ensure compliance with Article 6.02
Employees seeking a replacement issue of an item of industrial clothing on the grounds of loss, damage or theft shall submit a statutory declaration to the company setting out the facts and circumstances and in the event that it is agreed that a bona fide claim exists, the item will be replaced by the company at no cost to the employee.
Employees seeking reimbursement for a course shall apply in writing for reimbursement by August 1st for courses taken during the Fall or December 15th for courses taken in the Spring. Employees shall submit requests for reimbursement for summer session courses by May 1st.
