Enrollment Restriction. No person is otherwise eligible to enroll hereunder whose AvMed coverage was previously terminated for non-payment of Premium or cause, except with AvMed’s written approval.

Enrollment Restriction. No individual is eligible to enroll for coverage under this Contract whose AvMed coverage was previously terminated for non-payment of Premium, except with XxXxx’s written approval. If your coverage was previously terminated for non-payment of Premiums you are not eligible to enroll for coverage under this Contract without paying to us all past due Premiums under the Contract applicable in the previous 12 months, and any other amounts owed to us, including the first month Premium prepayment due under this Contract. No individual whose AvMed coverage was previously terminated for cause is eligible to enroll for coverage under this Contract.

Enrollment Restriction. Any Consultant that has their Consultant Agreement terminated, whether voluntarily or involuntarily, will not be eligible to re-enroll as a Consultant with Xxxxxxxxx for a period of 12 months. This 12-month restriction applies to the Consultant as well as those in their household.
Enrollment Restriction. Enrollment may proceed to the plan's maximum total number of Members unless restricted by DHS.
