Environmental Modifications. Physical adaptations to the home of the Enrollee or the Enrollee’s Family as required by the Enrollee’s LTSS Care Plan, that are necessary to ensure the health, welfare, and safety of the Enrollee or that enable the Enrollee to attain, maintain, or retain capability for independence or self-care in the home and to avoid institutionalization, and are not covered or available under any other funding source.
