ESSENTIAL COMMUNITY. PROVIDER
(1) Federally Qualified Health Centers (FQHCs) and FQHC "Look- Alike" clinics; (2) ▇▇▇▇ ▇▇▇▇▇ HIV/AIDS Program Providers; (3) Family Planning Providers; (4) Indian Health Providers; (5) Hospitals; and (6) Other ECP Providers including STD clinics, TB clinics, Hemophilia treatment centers, Black Lung clinics and other entities that serve predominately low-income, medically underserved individuals.
ESSENTIAL COMMUNITY. Supports (ECS) means state-funded services in the community pursuant to Minnesota Statutes, § 256B.0922, for persons not eligible for long-term care waiver services or Nursing Facility services for persons determined ineligible for EW or NF services during 2015 as a result of changes to Nursing Facility Level of Care. See section 2.86.
ESSENTIAL COMMUNITY. PROVIDER
