You may obtain an estimate of the cost for particular services from Participating Physicians and Providers by contacting AvMed’s Member Engagement Center at the telephone number on the cover of this Contract or on your AvMed Identification Card. The fact that we may provide you with such information does not mean that the particular service is a Covered Service. All terms and conditions included in your Contract apply.
