Primary Care Physician Selection. With the AvMed Engage Plan, Members are required to select a PCP upon enrollment. Upon enrollment, you can choose from a list of PCPs who are AvMed Engage Plan Participating Physicians. If you do not choose a PCP yourself, AvMed will select one for you.

Primary Care Physician Selection. With the AvMed Empower Plan, each Member may select a PCP upon enrollment, but is not required to do so. Although you have the option to select any provider you choose, we encourage you to select and develop a relationship with a PCP.
Primary Care Physician Selection. The Contractor will implement and maintain DHS approved procedures to ensure that each Member is allowed to select or change a Primary Care Physician from the Contractor's network of providers. The Contractor will assist Members in making their selection within thirty (30) days of their effective date of Enrollment. The Contractor will provide the Member sufficient information (verbal and written) in the appropriate language and reading level about the selection process and the available 109 Xxxxxx Medical Centers 95-23637 Article VI providers in the network to ensure their ability to make an informed decision.

  • Physician Visits This plan covers the services of a physician or other provider in charge of your medical care while you are inpatient in a general or specialty hospital.

  • Family Care and Medical Leave An unpaid Family Care and Medical Leave shall be granted, to the extent of and subject to the restrictions as set forth below, to an employee who has been employed for at least twelve (12) months and who has served for 130 workdays during the twelve (12) months immediately preceding the effective date of the leave. For purposes of this Section, furlough days and days worked during off-basis time shall count as "workdays".
