Evaluations of Individuals Served. 1) At a minimum, the Contractor must assess the mental health and addiction needs and make recommendations for all individuals presenting for service regardless of their ability to pay. 2) For all individuals meeting the eligibility criteria, this assessment must be comprehensive and an integral part of the treatment planning for the individual. This assessment must include the State-approved assessment instrument appropriate to the Individual; i.
