Common use of Exclude Yourself from the Settlement Clause in Contracts

Exclude Yourself from the Settlement. If you do not wish to take part in the Settlement, you may exclude yourself by sending to the Settlement Administrator a written “Request for Exclusion from the Class Action Settlement” letter or card postmarked no later than <<RESPONSE DEADLINE>>, with your name, address, telephone number, last four digits of your social security number, and your signature.

If you do not wish to take part in the class portion of the Settlement, you may exclude yourself by sending to the Settlement Administrator a written an executed “Request for Exclusion from the Class Action SettlementExclusion,” which is a letter or card postmarked no later than <<RESPONSE DEADLINE>>, postcard including with your name, address, telephone number, last four digits of your social security number, and your signature, and a statement that you wish to be excluded from the class Settlement. Any Request for Exclusion must be postmarked no later than <<RESPONSE DEADLINE>>.
