Common use of Extended Family Member Approval Clause in Contracts

Extended Family Member Approval. The employee must obtain approval of the department director before an absence may be paid as family member medical leave for an extended family member. The supervisor and department director will determine approval of the employee’s paid leave for extended family member illness at the time they process the employee’s payroll timecard. Written verification from the family member’s health care provider is required for any paid leave for extended family member illness. The health care provider’s written verification must state that the extended family member has an illness that requires the employee’s attendance. The supervisor may require alternative scheduling of non-emergency requests by the employee for particular times of paid leave for family member illness.
