Eyeglass Replacement Clause Samples

Eyeglass Replacement. An employee whose eyeglasses are broken or lost in the performance of duty shall submit a written report before the end of his duty shift explaining what caused said glasses to break. The County shall provide safety lens, scratch resistance and tinting as requested by the employee as replacement lens. Replacement frame cost shall not exceed two hundred fifty ($250) dollars.
Eyeglass Replacement. The Employer will reimburse an employee for replacement of lens(es) or frame broken during the performance of one’s duties. Replacement is to be of similar quality to those which had been broken. Reimbursement shall be limited to two hundred dollars ($200) per pair of eyeglasses and shall only be paid if the eyeglasses are not otherwise replaced by the employee’s insurance.
Eyeglass Replacement. The City agrees to reimburse members of the bargaining unit for prescription eyeglasses broken or damaged during the course of employment up to $200 for the repair or replacement of frames and lenses. Members may select more expensive eyeglasses by paying the additional cost.
Eyeglass Replacement. An employee whose glasses are broken on the job through no negligence of the employee shall be entitled to a replacement allowance not to exceed two hundred and fifty dollars ($250) per year. Incident reports must be filed and replacement approved in advance. All required corresponding receipts must be submitted to the Superintendent of Highways prior to reimbursement.
Eyeglass Replacement. Once per year (per employee), the CITY shall repair or replace eyeglasses damaged due to the employee's work provided that: 1. The damage was not due to negligence on the employee's part; and 2. The damage was not due to a defect(s) or wear on the glasses. City-paid repair or replacement shall be limited to a maximum of one-hundred and fifty dollars ($150) per incident, per year.
