Family Health Plus Populations. All Eligible Persons who meet the criteria listed in Section 369-ee of the SSL and/or New York State’s Operational Protocol for the Partnership Plan and who reside in the Contractor’s service area, as specified in Appendix M of this Agreement, shall be eligible for Enrollment in the Contractor’s Family Health Plus product.
