Fixed prosthetics. The artificial replacement of missing teeth with a crown or bridge. PART “C” - ORTHODONTICS Part C covers those services required to straighten abnormally arranged teeth. Pre-approval by the benefits carrier is necessary. The Plan will pay 60% of: Braces Up to a lifetime maximum of $2,750 per person. Costs of lost or stolen braces are not eligible for payment. To be eligible for orthodontic services, the employee must have been enrolled in this dental benefit for twelve (12) months.
