Foreseeable Disability. If the teacher knows, or reasonably should know, that he has a physical or mental condition, which will result in disability, the teacher shall as soon as reasonable: A. Notify the Board as to the nature and extent of the expected disability in accordance with this Section. B. Furnish the Board a statement from the attending physician specifying in the physician’s opinion: 1. Any limitations on the performance of duties; 2. The probable date when the teacher will be significantly impaired in the performance of his duties; and 3. The probable length of time, if any, during which the teacher will be disabled from performing his work assignments. C. Furnish the Board such information, as the Board shall determine, including the attending physician's release, to assure the safety and welfare of the teacher, students, and other employees
