Frailty. The NWL ambition is to establish acute frailty services in emergency departments across NW London with the aims of identifying and managing older frail patients who require specialised frailty support as soon as possible. This will ensure frail patients are not admitted unnecessarily and be supported at home with full wrap around services. Target is to reduce 75+ rate of admission by 3%. The Trust is looking to strengthen its MDT approach in the Emergency Departments, working with system partners to enable more people to go home directly from ED and therefore to avoid unnecessary admissions to an acute bed. We are currently piloting a number of different models and pathways particularly focused on older, frail patients that will inform how we develop a more dedicated front door service for winter and beyond. Therapies pilot for fallers – West Mid The aim of the pilot which commenced in August 2019, is to prevent inappropriate admissions by earlier identification and therapy assessment with the ED department at West Mid. The pilot is staffed by a therapist 8am-4pm Monday to Friday which has been provided from within the existing staffing envelope. The model sees patients identified as a xxxxxx within 30 minutes and a full functional assessment undertaken, and initial results have identified that this has increased discharges from ED, avoiding admissions.

  • Hazing Hazing is a process, based on tradition that is used by groups to maintain a hierarchy (i.e., a pecking order) within the group. Regardless of consent, the rituals require individuals to engage in activities that are physically and/or psychologically stressful. These activities can be humiliating, demeaning, intimidating, and exhausting, all of which results in physical or emotional discomfort. Hazing is about group dynamics and proving one’s worthiness to become a member of a specific group. The newcomer, or victim, is hazed. Once accepted by the group, the victim may become a bystander, watch others get hazed, achieve senior status, and ultimately become a perpetrator of hazing. In New Jersey, hazing is a separate criminal offense under N.J.S.A. 2C:40-3. School officials should be aware that hazing which involves the participation of a coach or a teacher may also constitute child abuse. Hazing may also involve other predicate crimes, such as sexual assault. Hazing may also constitute HIB. As noted in Article 4.4.2, although HIB is not a separately defined criminal offense, the conduct that constitutes bullying may constitute one or more criminal or disorderly person offenses, including assault, harassment, threats, robbery and sexual offenses.

  • Trees 1) Any major tree surgery or tree removal over a three (3) inch diameter, which becomes necessary through no fault of the Contractor, will be at the County's expense.

  • Cemetery The Local Church agrees and insures that, after the Disaffiliation Date, its cemetery and/or columbarium, will continue to be maintained in substantially the same manner as presently maintained. In addition, Local Church will honor any and all contracts, deeds, and agreements for burial and/or internment in its cemetery or columbarium, as well as insuring and continued access for families and loved ones of United Methodists buried there and for burials in unfilled xxxxxx and columbarium slots (including granting an access easement to the Conference and members of the United Methodist Church for visitations, historical research, and related purposes).

  • Housekeeping Service and Cleanliness Housekeeping Service will be provided to clean only the kitchenette and washroom areas of the Room on a regularly scheduled basis as posted by the Manager. This Housekeeping Service is not optional. Residents must ensure that all counters and sinks are clear of any dishes, appliances, or any other personal items. As part of the Housekeeping Service, the Room will be inspected. Should the Room be found in an unhygienic state, the Resident(s) will be given 24 hours to return it to a proper state, or it will be cleaned at the discretion of the Manager with a minimum charge of $25.00. The Resident shall, at all times during the Term, keep all parts of the Room clean, including and not limited to; floor coverings, doors, walls, ceilings, kitchenette appliances, counters, cupboards, faucets, sinks, furniture, glass, window frames, and other furnishings. Vacuums, brooms, and mops may be signed out at the front desk, subject to availability.

  • Livestock 19.1. Except with the prior written consent of the Council the Tenant shall not keep any animals or livestock on the Allotment site save rabbits and hens (no Cockerels) to the extent permitted by section 12 Allotments Act 1950. (Such animals not to be kept for trade or business purposes and accordingly to be limited in number as the Council may provide in writing.)

  • Cutting, Patching, and Fitting The Contractor shall do all cutting, patching, and fitting of the Work that may be required to make its several parts come together properly and fit.

  • Cleanliness Resident agrees to keep the room in a clean and sanitary condition during the entire term of occupancy and to return his/her room to Provider in the same condition, including general cleanliness, as it was at the beginning of the term of occupancy, normal wear and tear excepted as determined by College acting on behalf of Provider. When there is vacant space in a partially occupied multi-person room, the vacant space must be maintained by the occupant(s) in a manner that will allow a new Resident to move in immediately. Failure to do so will result in assessment of a cleaning charge to Resident’s account.
