It will integrate health and social care, and will be able to navigate and, ideally, schedule appointments with other services. Front Door schemes Evidence suggests that the longer patients wait in the Emergency Department the greater risk there is to morbidity and mortality, and that ‘boarding’ (patients remaining in the department whilst waiting for a suitable inpatient bed to become available) is likely to increase length of stay, detract from overall patient experience and risk breakdown in communications because of the number of hand offs/transfers involved. Patients waiting in the Emergency Department for an inpatient bed also reduce the capacity in the department to see and treat patients, resulting in longer waiting times in the department and impacting on the ability of ambulance teams to unload patients. A number of schemes will therefore be in place for the winter period to maintain patient flow through, and safety in, the Emergency Department. This includes an increased ambulatory care offering, providing an alternative setting to care in the Emergency Department, initiatives to ensure ambulance handover targets are maintained, and more robust plans for managing the increasing number of patients experiencing long lengths of stay whilst awaiting mental health beds.
