Common use of Hospice Patient Clause in Contracts

Hospice Patient. a patient diagnosed or referred by a physician, to a Hospice and who alone, or in conjunction with designated family members, has requested and received admission into a hospice program. Written certification by the patient's Doctor that the Hospice Patient has a life expectancy of 6 months or less is required.
