Identification Cards. Identification (“ID”) cards are issued by Us for identification purposes only. Possession of any ID card confers no right to services or benefits under this Contract. To be entitled to such services or benefits, Your Premiums must be paid in full at the time that the services are sought to be received.

Identification Cards. Cards issued by AvMed to Members pursuant to this Contract are for purposes of identification only. Possession of an AvMed Identification Card confers no right to Health Care Services or other benefits under this Contract. To be entitled to such services or benefits the holder of the card must be, in fact, a Member on whose behalf all applicable Premiums under this Contract have actually been paid and accepted by AvMed. Please carry your Identification Card with you at all times, and present it before Covered Services are rendered. If your Identification Card is missing, lost, or stolen, contact XxXxx’s Member Engagement Center at 0-000-000-0000, or visit AvMed’s website at xxx.xxxxx.xxx. Member Identification Cards are AvMed’s property and, upon request, will be returned to AvMed within 30 days of the termination of your coverage.

Identification Cards. Picture identification cards may be issued to employees by the City, and if so, shall be worn in a sensible, but conspicuous place on their person by all such employees or as reflected in the current practice of the department. Any such picture identification cards shall identify the employee by name, department and photograph, consistent with the practice of each department. The cost of replacing the card damaged due to normal wear and tear shall be borne by the City.

Identification Cards. The College/University shall issue official identification cards to all adjunct faculty at no cost. Cost for replacement shall be in accordance with College/University policies and practices.

The Contractor also agrees to issue an identification card for its members to use when obtaining Covered Services. The card may identify the holder as a RIte Care, Rhody Health Partners, or an ACA Adult Expansion Population member using an alpha or numeric indicator but should not be overtly different in design from the card issued to other enrolled groups. The Contractor agrees to issue all Medicaid Managed Care members a permanent identification card within ten (10) calendar days after receiving notification from EOHHS of their enrollment. The card must include at least the following information: • Health Plan name • Twenty-four-hour Health Plan telephone number for use in urgent or emergent medical situations • Telephone number for member Services function (if different) • Telephone Number for Behavioral Health Services
