Immediate Reporting Requirements. The Supplier shall notify the Company of any unresolved Healthworks program issue in a method provided for and approved by the Company, particularly as it relates to system coordination or access to essential services. The Supplier program supervisor shall also will immediately notify the assigned caseworker as identified in SACWIS of any significant events, changes in family circumstances, or unusual incidents involving the Client or family members including but not limited to incidents of suspected abuse or neglect which have been or are to be reported to Child Abuse Hotline, police involvement/intervention with the family, major health problems or death in the immediate family, emotional/mental/physical deterioration, change in household composition, change in residence, suspected drug or alcohol abuse, any circumstances or incident which poses a threat to the safety and well-being of any involved child or would post such a threat if the child were in the current custody of the parent/client, or other significant information or changes in family circumstances. Any Supplier doing business with DCFS is considered a mandated reporter of child abuse or neglect.
