Immunization Records. Immunization Compliance Form (Form 121) from the Health Department or physician. This is a specific form required to be in the student's cumulative record showing proof that the required number of immunizations have been received.
Immunization Records. Immunization records meeting the requirements of the state and School must be turned in with the application. St. Mark’s School does not accept Immunization Exemption Affidavits and students will not be able to attend school unless all immunizations are current.
Immunization Records. At the start of each academic year students are required to have on record all childhood and current immunizations. (e.g.: DTap, IPV, MMR, HepB, COVID-19, and influenza) and a negative two-step TB test. Students will not be permitted to start clinical experiences without this documentation. Students must also provide either documentation of having had varicella (VAR/chicken pox) or having been vaccinated for such condition. If the student had varicella and did not see physician, verification must be achieved by having a titer (blood test) completed by the physician.

Immunization Records. All families are required to turn in current immunization. Immunization records are due the 1st of January, April 1st, July 1st Oct 1st and Dec 1st. IF your child is currently on their immunization the daycare will ask you for a note from the child’s health care provider. If your child is not currently immunized they will not be accepted into the daycare.

Daily Routine/ Monthly Activities/Snack Schedule Daily routine and snack schedule will be posted at the centre. We have different themes planned for each month and a set of varying daily activities that build towards the theme of the month. We use an application called BrightWheels [ available for download from the Google Play Store or Apple App Store (iOS) ] on which pictures of varying activities done by your child will be sent and can be viewed on your mobile smartphone. Immunization Records. Current students who have turned in a completed Intent to Return form before the applicable deadline will need to deliver the documentation described above during registration. Current students who have not completed registration by June 30 may lose their space and are not guaranteed admission based on enrollment after that date. Any parent or guardian who does not intend to re-enroll his or her child at Xxxx Pubic Charter School is asked to properly withdraw him or her from the School before June 30 to alleviate administrative burden on the School.
