Common use of Individually Identifiable Clause in Contracts

Individually Identifiable. Health Information is health information, including demographic information collected from an individual, that: (i) Is created or received by a health care provider, health plan, employer, or health care clearinghouse; (ii) Relates to the past, present, or future physical or mental health or condition of an individual; the provision of health care to an individual; or the past, present, or future payment for the provision of health care to an individual; and (iii) Either identifies the individual or with respect to which there is a reasonable basis to believe the information can be used to identify the individual.

Individually Identifiable. Health Information is means health information, in any form or medium, including demographic information collected from an individual, that: (i) Is that is created or received by a health care provider, health plan, employer, covered entity or health care clearinghouse; (ii) Relates a business associate of the covered entity and relates to the past, present, or future physical or mental care of the individual. Individually identifiable health or condition of an individual; the provision of health care to an individual; or the past, present, or future payment for the provision of health care to an individual; and (iii) Either information is information that identifies the individual directly or with respect to which there is a reasonable basis to believe the information can be used to identify the individual. Individually Identifiable. Health Information is a subset of health information, including demographic information collected from an individual, that: (i) Is created or received by a health care provider, health plan, employer, or health care clearinghouse; (ii) Relates individual and relates to the past, present, or future physical or mental health or condition of an individual; the provision of health care to an individual; or the past, present, present or future payment for the provision of health care to an individual; and (iii) Either identifies the individual , as set forth in 45 CFR  164.501 as currently enacted and subsequently amended or with respect to which there is a reasonable basis to believe the information can be used to identify the individualrevised.

Individually Identifiable. Health Information means information that is a subset of health information, including demographic information collected from an individualIndividual, that: and: (ia) Is is created or received by from a health care provider, health plan, employer, employer or health care clearinghouse; ; and (iib) Relates relates to the past, present, present or future physical or mental health or condition of an individual; Individual, the provision of health care to an individual; a patient, or the past, present, or future payment for the provision of health care to an individualIndividual; and and (iiii) Either identifies the individual patient; or (ii) with respect to which there is a reasonable basis to believe that the information can be used to identify the individualpatient.
