Information Concerning the Maintenance of Applicant’s Facilities. Promptly after the issuance of a license, Applicant shall provide SWBT with the name, title, business address, and business telephone number of the manager responsible for routine maintenance of Applicant’s facilities and shall thereafter notify SWBT of changes to such information. The manager responsible for routine maintenance of Applicant’s facilities shall, on SWBT’s request, identify any contractor, subcontractor, or other person performing maintenance activities on Applicant’s behalf at a specified site.

The manager responsible for routine maintenance of Applicant’s facilities shall, on NEVADA’s request, identify any contractor, subcontractor, or other person performing maintenance activities on Applicant’s behalf at a specified site.
