Initial Failure. The employee shall be notified that Wellspark continues to report them as noncompliant with the EOs. Not later than close of business Tuesday, October 12, 2021, said employee shall provide documentation to the Agency HR Business Partner (hereinafter “HRBP”) or designee that they are either fully vaccinated or testing for COVID 19 as prescribed by the EOs. For those employees not offered the option to test if they are not vaccinated, compliance shall also include proof of the first dose of a two- dose vaccination series with a second dose scheduled and weekly testing up to14 days after the second dose and a negative test in the last 72 hours.
