Common use of Issuance of 1099 Forms Clause in Contracts

Issuance of 1099 Forms. After this Agreement has been executed and the settlement funds have been transmitted to Davia’s counsel, Exclusive Imports shall issue three separate 1099 forms, as follows: (a) The first 1099 shall be issued to the Office of Environmental Health Hazard Assessment, X.X. Xxx 0000, Xxxxxxxxxx, XX 00000 (EIN: 00-0000000) in the amount paid pursuant to Sections 4.1 and 4.2; (b) The second 1099 shall be issued to Davia in the amount paid pursuant to Sections 4.1 and 4.2, whose address and tax identification number shall be furnished upon request; and (c) The third 1099 shall be issued to the Xxxxxxx Law Firm (EIN: 55-08-58910) in the amount paid pursuant to Section 4.2 and Section 4.3.

Issuance of 1099 Forms. After this Agreement has been executed and the settlement funds have been transmitted to Davia’s counsel, Exclusive Imports Vinylux shall issue three separate 1099 forms, as follows: (a) The first 1099 shall be issued to the Office of Environmental Health Hazard Assessment, X.X. Xxx 0000, Xxxxxxxxxx, XX 00000 (EIN: 00-0000000) in the amount paid pursuant to Sections 4.1 and 4.2; (b) The second 1099 shall be issued to Davia in the amount paid pursuant to Sections 4.1 and 4.2, whose address and tax identification number shall be furnished upon request; and (c) The third 1099 shall be issued to the Xxxxxxx Law Firm (EIN: 55-08-58910) in the amount paid pursuant to Section 4.2 and Section 4.3.

Issuance of 1099 Forms. After this Agreement has been executed and the settlement funds have been transmitted to Davia’s counsel, Exclusive Imports Earth Pak shall issue cause three separate 1099 formsforms to be issued, as follows: (a) The first 1099 shall be issued to the Office of Environmental Health Hazard Assessment, X.X. Xxx 0000, Xxxxxxxxxx, XX 00000 (EIN: 00-0000000) in the amount paid pursuant to Sections 4.1 and 4.2; (b) The second 1099 shall be issued to Davia in the amount paid pursuant to Sections 4.1 and 4.2, whose address and tax identification number shall be furnished upon request; and (c) The third 1099 shall be issued to the Xxxxxxx Law Firm (EIN: 55-08-58910) in the amount paid pursuant to Section 4.2 4.3 and Section 4.34.2.

After this Agreement has been executed and the settlement funds have been transmitted to Davia’s counsel, Exclusive Imports Sunham shall issue cause three separate 1099 formsforms to be issued, as follows: (a) The first 1099 shall be issued to the Office of Environmental Health Hazard Assessment, X.X. Xxx 0000, Xxxxxxxxxx, XX 00000 (EIN: 00-0000000) in the amount paid pursuant to Sections 4.1 and 4.2; (b) The second 1099 shall be issued to Davia in the amount paid pursuant to Sections Sections 4.1 and 4.2, whose address and tax identification number shall be furnished upon request; and (c) The third 1099 shall be issued to the Xxxxxxx Law Firm (EIN: 55-08-58910) in the amount paid pursuant to Section 4.2 4.3 and Section 4.34.2.

Xxx 0000, Xxxxxxxxxx, XX 00000 (EIN: 00-0000000) in the amount paid pursuant to Sections 4.1 and 4.2; (b) The second 1099 shall be issued to Davia in the amount paid pursuant to Sections Sections 4.1 and 4.2, whose address and tax identification number shall be furnished upon request; and (c) The third 1099 shall be issued to the Xxxxxxx Law Firm (EIN: 55-08-58910) in the amount paid pursuant to Section 4.2 4.3 and Section 4.34.2.

Xxx 0000, Xxxxxxxxxx, XX 00000 (EIN: 00-0000000) in the amount paid pursuant to Sections 4.1 and 4.2; (b) The second 1099 shall be issued to Davia in the amount paid pursuant to Sections 4.1 and 4.2, whose address and tax identification number shall be furnished upon request; and (c) The third 1099 shall be issued to the Xxxxxxx Law Firm (EIN: 55-08-58910) in the amount paid pursuant to Section 4.2 4.3 and Section 4.34.2.

Xxx 0000, Xxxxxxxxxx, XX 00000 (EIN: 00-0000000) in the amount paid pursuant to Sections 4.1 and 4.24.2 (if any); (b) The second 1099 shall be issued to Davia in the amount paid pursuant to Sections Sections 4.1 and 4.24.2 (if any), whose address and tax identification number shall be furnished provided to Xxxx upon request; and (c) The third 1099 shall be issued to the Xxxxxxx Law Firm (EIN: 55-08-58910) in the amount paid pursuant to Section 4.3 and 4.2 and Section 4.3(if any).

Issuance of 1099 Forms. After this Agreement has been executed and the settlement funds have been transmitted to Davia’s counsel, Exclusive Imports Keeco shall issue cause three separate 1099 formsforms to be issued, as follows: (a) The first 1099 shall be issued to the Office of Environmental Health Hazard Assessment, X.X. Xxx 0000, Xxxxxxxxxx, XX 00000 (EIN: 00-0000000) in the amount paid pursuant to Sections 4.1 and 4.2; (b) The second 1099 shall be issued to Davia in the amount paid pursuant to Sections 4.1 and 4.2, whose address and tax identification number shall be furnished upon request; and (c) The third 1099 shall be issued to the Xxxxxxx Law Firm (EIN: 55-08-58910) in the amount paid pursuant to Section 4.2 Sections 4.3 and Section 4.34.2.

Issuance of 1099 Forms. After this Agreement has been executed and the settlement funds have been transmitted to Davia’s counsel, Exclusive Imports Berkshire Blanket shall issue cause three separate 1099 formsforms to be issued, as follows: (a) The first 1099 shall be issued to the Office of Environmental Health Hazard Assessment, X.X. Xxx 0000, Xxxxxxxxxx, XX 00000 (EIN: 00-0000000) in the amount paid pursuant to Sections 4.1 and 4.2; (b) The second 1099 shall be issued to Davia in the amount paid pursuant to Sections 4.1 and 4.2, whose address and tax identification number shall be furnished upon request; and (c) The third 1099 shall be issued to the Xxxxxxx Law Firm (EIN: 55-08-58910) in the amount paid pursuant to Section 4.2 4.3 and Section 4.34.2.

After this Agreement has been executed and the settlement funds have been transmitted to Davia’s counsel, Exclusive Imports Alliance shall issue three separate 1099 forms, as follows: (a) The first 1099 shall be issued to the Office of Environmental Health Hazard Assessment, X.X. Xxx 0000, Xxxxxxxxxx, XX 00000 (EIN: 00-0000000) in the amount paid pursuant to Sections 4.1 and 4.2; (b) The second 1099 shall be issued to Davia in the amount paid pursuant to Sections 4.1 and 4.2, whose address and tax identification number shall be furnished upon request; and (c) The third 1099 shall be issued to the Xxxxxxx Law Firm (EIN: 55-08-58910) in the amount paid pursuant to Section 4.2 and Section 4.3.

Issuance of 1099 Forms. After this Agreement agreement has been executed and the settlement funds have been transmitted to Davia’s counsel, Exclusive Imports Sur La Table shall issue three separate 1099 forms, as follows: (a) The first 1099 shall be issued to the Office of Environmental Health Hazard Assessment, X.X. Xxx 0000, Xxxxxxxxxx, XX 00000 (EIN: 00-0000000) in the amount paid pursuant to Sections 4.1 and 4.2; (b) The second 1099 shall be issued to Davia in the amount paid pursuant to Sections 4.1 and 4.2, whose address and tax identification number shall be furnished upon request; and (c) The third 1099 shall be issued to the Xxxxxxx Law Firm (EIN: 55-08-58910) in the amount paid pursuant to Section 4.2 and Section 4.3.

Issuance of 1099 Forms. After this Agreement has been executed and the settlement funds have been transmitted to Davia’s counsel, Exclusive Imports Dick’s shall issue cause three separate 1099 formsforms to be issued, as follows: (a) The first 1099 shall be issued to the Office of Environmental Health Hazard Assessment, X.X. Xxx 0000, Xxxxxxxxxx, XX 00000 (EIN: 00-0000000) in the amount paid pursuant to Sections 4.1 and 4.2; (b) The second 1099 shall be issued to Davia in the amount paid pursuant to Sections Sections 4.1 and 4.2, whose address and tax identification number shall be furnished upon request; and (c) The third 1099 shall be issued to the Xxxxxxx Law Firm (EIN: 55-08-58910) in the amount paid pursuant to Section 4.2 4.3 and Section 4.34.2.

Xxx 0000, Xxxxxxxxxx, XX 00000 (EIN: 00-0000000) in the amount paid pursuant to Sections 4.1 and 4.2; (b) The second 1099 shall be issued to Davia in the amount paid pursuant to Sections 4.1 and 4.2, whose address and tax identification number shall be furnished upon request; and (c) The third 1099 shall be issued to the Xxxxxxx Law Firm (EIN: 55-08-58910) in the amount paid pursuant to Section 4.2 4.3 and Section 4.34.2.

Issuance of 1099 Forms. After this Agreement has been executed and the settlement funds have been transmitted to Davia’s counsel, Exclusive Imports Container Store shall issue three separate 1099 forms, as follows: (a) The first 1099 shall be issued to the Office of Environmental Health Hazard Assessment, X.X. Xxx 0000, Xxxxxxxxxx, XX 00000 (EIN: 00-0000000) in the amount paid pursuant to Sections 4.1 and 4.24.2 (if any); (b) The second 1099 shall be issued to Davia in the amount paid pursuant to Sections 4.1 and 4.24.2 (if any), whose address and tax identification number shall be furnished upon request; and (c) The third 1099 shall be issued to the Xxxxxxx Law Firm (EIN: 55-08-58910) in the amount paid pursuant to Section 4.2 (if any) and Section 4.3.

Xxx 0000, Xxxxxxxxxx, XX 00000 (EIN: 00-0000000) in the amount paid pursuant to Sections 4.1 and 4.2; (b) The second 1099 shall be issued to Davia in the amount paid pursuant to Sections 4.1 and 4.2, whose address and tax identification number shall be furnished upon request; and (c) The third 1099 shall be issued to the Xxxxxxx Law Firm (EIN: 55-08-58910) in the amount paid pursuant to Section 4.2 4.3 and Section 4.34.2.

Xxx 0000, Xxxxxxxxxx, XX 00000 (EIN: 00-0000000) in the amount paid pursuant to Sections 4.1 and 4.2; (b) The second 1099 shall be issued to Davia in the amount paid pursuant to Sections 4.1 and 4.2, whose address and tax identification number shall be furnished upon request; and (c) The third 1099 shall be issued to the Xxxxxxx Law Firm (EIN: 55-08-58910) in the amount paid pursuant to Section 4.2 4.3 and Section 4.34.2.
